Endoscopic transsphenoidal surgery reduces the need for re-operation compared with the microscopic approach in pituitary macroadenomas

Shimony N, Popovits N, Shofty B, et al. - In recent years, a shift in pituitary surgery has been observed from microscopic surgery (MS) to endoscopic endonasal surgery (EES). Researchers here described their experience in a high-volume referral center experienced in both endoscopic and microscopic approaches to compare the necessity for re-operation after initial resection of non-functioning pituitary macroadenomas utilizing these surgical approaches. At their institution, trans-sphenoidal pituitary surgery was performed on 684 patients (398 with NF adenomas) between 2006 and 2017. Of these, 87 newly diagnosed patients with non-functioning pituitary macroadenomas (NFPMA; 48-microscopic and 39-endoscopic) had complete follow-up (mean 72 months, minimum 2 years) available. Per findings, there was a lower need for re-operation in correlation with performing EES in NFPMA compared with the MS approach, with a similar rate of extent of resection and complications.
