Endoscopic therapy and surveillance vs esophagectomy for early esophageal adenocarcinoma: A review of early outcomes and cost analysis

Wirsching A, et al. - Endoscopic therapy is thought to be comparable to esophagectomy in terms of oncologic outcomes in early (cT1) esophageal adenocarcinoma (EC), so researchers conducted this retrospective review comparing patients undergoing endoscopic vs surgical therapy for cT1 EC between 2010 and 2015, in terms of early outcomes and financial costs. Patients undergoing endoscopic therapy (N=20) and esophagectomy (N=23) were similar regarding age, BMI, and Charlson Comorbidity Scores. Esophagectomy was associated with a greater procedure specific morbidity and financial costs compared to endoscopic therapy. A significant variation in the cost of therapy was noted: endoscopic ($9K-128K) & surgical ($40K-248K).
