Endophthalmitis in advanced microbial keratitis: Risk factors and examination findings

Christopher KL, Hood CT, Mian SI, et al. - Researchers conducted a retrospective analytical study to assess clinical and ultrasonographic findings correlated with advanced microbial keratitis with concurrent endophthalmitis. The sample consisted of patients with severe microbial keratitis who had ocular ultrasonography for possible endophthalmitis at the institution over a 5-year period. For the 81 eyes that included, 15 were diagnosed with endophthalmitis based on clinical results and ultrasound evidence for vitritis. Diagnosis of endophthalmitis can be difficult in the eyes with advanced microbial keratitis, and visual outcomes are often extremely poor. Clinicians will have an especially high level of suspicion in eyes exhibiting light perception or no vision of light perception, cataract extraction history, full-thickness ulcer or perforation, or ultrasound choroidal thickening or retinal detachment.
