Endometrial hyperplasia and risk of coexistent cancer: WHO vs EIN criteria

Travaglino A, et al. - Through a search of electronic databases for relevant articles from inception to July 2018, researchers assessed which subjective classifications of endometrial hyperplasia (EH) (WHO [based on the subjective evaluation of cytologic atypia] or EIN [based on a combination of several parameters, assessable subjectively, or objectively through computerized analysis]) has a better prognostic ability, by evaluating the risk of coexistent cancer. The results seemed to suggest that WHO and subjective EIN system have comparable prognostic value; however, EIN criteria seems more sensitive and more suitable for choosing women who need treatment. On the other hand, WHO criteria, based on cytologic atypia, appears more specific for lesion at higher risk of cancer. Consequently, an integration of the EIN system with cytologic atypia ought to be considered.
