Clostridium difficile colitis following revision total knee arthroplasty: Incidence and risk factors

Curtis GL, et al. - Six thousand twenty-three revision total knee arthroplasty (rTKA) procedures from 2015-2016 were identified by the researchers in order to assess the incidence of Clostridium difficile-associated diarrhea (CDAD) within 30 days of rTKA and recognize perioperative risk factors for CDAD following rTKA. Twenty-four out of 6,023 (0.4%) patients were observed with CDAD within 30 days of rTKA. The CDAD rate after the aseptic revision was 0.2%, while the prevalence of CDAD post-septic revision was 1.1%. Statistically significant risk factors for CDAD post-rTKA were preoperative functional dependence, septic revision, and cancer. Hence, the incidence of CDAD post-rTKA in the United States was approximately 0.4%. Therefore, in high-risk patients, the prevention of CDAD should be taken into account pre-surgery and antibiotic selection for other infections should be carefully managed.
