Eligibility and preventive potential for new evidence-based cardiovascular drugs in secondary prevention

Mortensen MB, et al. - Given the efficacy of new therapies for mainly secondary prevention of atherosclerotic cardiovascular disease has been shown in 12 randomized clinical trials (RCTs) recently, but the existence of uncertainty and questions regarding the use of these RCT findings in real-world clinical practice has led researchers to inquire about the eligibility as well as the preventive potential for these novel preventive therapies in a contemporary population. Overall 6,292 patients with known ischemic heart disease (IHD) and 2,277 with a prior myocardial infarction (MI) were enrolled. They found that eligibility for 1 or more new medications was 80% in patients with IHD at baseline, and this value was 99% in patients with MI at baseline, by meeting RCT enrollment criteria. In patients with IHD or MI at baseline, the 5-year percentage of major cardiovascular events that could be averted for each novel therapy was estimated to be 1% to 20%. Overall, findings revealed that the eligibility criteria for additional new secondary prevention therapies were met by most patients with known IHD or previous MI. This fosters questions for the cardiovascular community and health care authorities regarding access to these potentially expensive therapies, including strategies for prioritizing their use.
