Elevated presepsin is associated with perioperative major adverse cardiovascular and cerebrovascular complications in elevated-risk patients undergoing noncardiac surgery: The Leukocytes and Cardiovascular Perioperative Events Study

Handke J, et al. - In patients with elevated cardiovascular risk, researchers determined if noncardiac surgery was related to alterations in atherogenic leukocyte subsets. They also investigated whether these changes were associated with perioperative major adverse cardiovascular and cerebrovascular events (MACCEs). In this single-center prospective observational cohort study including 40 patients, findings revealed a link between noncardiac surgery and an increase in atherogenic leukocyte subsets. An association of elevated preoperative presepsin with MACCE and improved N-terminal probrain natriuretic peptide (NT-proBNP)-based risk evaluation was revealed in a post hoc analysis. At a cutoff of 184 pg·mL−1 following validation in an independent data set, presepsin may complement NT-proBNP-based risk prediction, then increasing the share of correctly identified high-risk patients.
