Elevated ESR/CRP ratio is associated with re-infection following debridement, antibiotics and implant retention in chronic periprosthetic joint infections

Maier SP, Klemt C, Tirumala V, et al. - In this study, the diagnostic accuracy of erythrocyte sedimentation rate (ESR) in predicting postoperative reinfection was assessed in patients that had undergone debridement, antibiotics and implant retention (DAIR). A retrospective review was conducted to include a total of 179 consecutive patients that had undergone DAIR revision TJA for periprosthetic joint infections (PJI). Individuals were classified by acuity of their infection: acute PJI, acute hematogenous PJI, and chronic PJI. Researchers estimated area under the receiver operating characteristic curve to analyze ECR as diagnostic marker for prognosticating postoperative reinfection in patients that underwent DAIR. The data showed that increased ECR was correlated with an increased reinfection risk in patients that had undergone DAIR for chronic PJI, implying that preoperative ECR may be a beneficial prognosticator to distinguish patients at increased risk of reinfection following DAIR for chronic periprosthetic joint infections.
