Efficacy of systemic oncological treatments in patients with advanced esophageal or gastric cancers at high risk of dying in the middle and short term: An overview of systematic reviews

Santero M, Pérez-Bracchiglione J, Acosta-Dighero R, et al. - Via this overview of systematic reviews, researchers focused on the effectiveness of systemic oncological treatments (SOT) vs the best supportive care or placebo in patients suffering from advanced esophageal or gastric malignancies in an end-of-life context. Eligible systematic reviews (SRs) published from 2008 onwards were identified from MEDLINE, EMBASE, The Cochrane Library, Epistemonikos, and PROSPERO. Overall 16 SRs (19 included trials) were included. Whether SOT confers improved overall survival and progression- free survival over more conservative approaches remains uncertain owing to the very low certainty of evidence. Experts found that existing evidence is very uncertain regarding SOT effectiveness for advanced esophageal or gastric cancers. Thus, there is a necessity for high-quality SRs and further randomized clinical trials that include a thorough evaluation of patient-centered outcomes.
