Efficacy of prehospital administration of fibrinogen concentrate in trauma patients bleeding or presumed to bleed (FIinTIC): A multicentre, double-blind, placebo-controlled, randomized pilot study

Ziegler B, Bachler M, Haberfellner H, et al. - Given the substantial contribution of trauma-induced coagulopathy (TIC) to mortality in bleeding trauma patients, researchers conducted a prospective, randomized, placebo-controlled, double-blinded, international clinical trial with the aim to determine if administration of fibrinogen concentrate in the prehospital setting enhances blood clot stability in trauma patients bleeding or presumed to bleed. They included a total of 53 adult trauma patients with major bleeding and in need of volume therapy; of these, 28 were administered fibrinogen concentrate and 25 received placebo. Observations revealed a decrease in median maximum clot firmness in the FIBTEM assay in the placebo group between baseline (before administration of study treatment) and admission to the Emergency Department, whereas FC Group showed its increase. Findings overall support the feasibility of early fibrinogen concentrate administration in the complex and time-sensitive environment of prehospital trauma care. It was identified as protective against early fibrinogen depletion, and led to promotion of rapid blood clot initiation and clot stability.
