Efficacy and safety of corticosteroids for septic shock in immunocompromised patients: A cohort study from MIMIC

Lu X, Wang X, Gao Y, et al. - Researchers conducted a cohort study of immunocompromised patients with septic shock using the Medical Information Mart for Intensive Care III (MIMIC-III) database in order to determine the efficacy and safety of corticosteroids in these patients. The database yielded data from 866 patients for inclusion in this study; of these, 395 had exposure to intravenous corticosteroids and 471 did not. The immunocompromised population with septic shock exhibited no improved mortality or hemodynamic stability in correlation with receipt of corticosteroid therapy for the maintenance of blood pressure. For the subgroup of patients with metastatic cancer, a statistically significant increase in the 30-day mortality risk was observed in correlation with corticosteroids infusion. They observed adverse consequences in terms of hemodynamic stability, prolonged ICU and hospital duration, and increased risk of hyperglycemia in correlation with corticosteroids.
