Effects of the medial plateau bearing insert conformity on mid-flexion paradoxical motion in a posterior stabilized total knee arthroplasty design

Khasian M, Meccia BA, LaCour MT, et al. - The objective of this study is to investigate the biomechanical effect of increasing bearing conformity on a posterior-stabilized (PS) total Knee Arthroplasty (TKA). The sagittal conformity of the medial plateau of a PS TKA design was altered applying a validated computational model of the knee joint. Three scenarios were developed and assessed for mechanics: baseline conformity, increased conformity, and decreased conformity. For PS TKA designs, elevating medial conformity of the bearing insert seems to decrease mid-flexion sliding, although this comes at the expense of increased femorotibial forces. As per the findings, this could be due to kinematic conflicts that may be introduced with highly constraining designs.
