Effects of the medial plateau bearing insert conformity on mid-flexion paradoxical motion in a posterior-stabilized total knee arthroplasty design

Khasian M, Meccia BA, LaCour MT, et al. - This research was undertaken to explore the biomechanical impact of increasing bearing conformity on a posterior-stabilized (PS) total knee arthroplasty (TKA). The sagittal conformity of the medial plateau of a PS TKA design was altered using a validated computational model of the knee joint. Researchers assessed and created three scenarios for mechanics: baseline conformity, increased conformity, and decreased conformity. It has been reported that increasing medial conformity of the bearing insert seems to decrease mid-flexion sliding for PS TKA designs, although this comes at the expense of increased femorotibial forces. It has been reported that this could be due to kinematic conflicts that may be introduced with highly constraining designs.
