Effects of severe hypoglycemia on cardiovascular outcomes and death in the Veterans Affairs Diabetes Trial

Davis SN, et al. - In the Veterans Affairs Diabetes Trial (VADT), researchers identified risk factors for severe hypoglycemia and assessed the associations between severe hypoglycemia, severe cardiovascular adverse events, and cardiovascular and all-cause mortality. This post hoc analysis of VADT data included 1,791 military veterans (age 60.5 ± 9.0 years) with suboptimally controlled type 2 diabetes (HbA1c 9.4 ± 2.0%) with or without a known cardiovascular disease duration of 11.5 ± 7.5 years and additional cardiovascular risk factors. In multivariable analysis, insulin use at baseline, proteinuria, and autonomic neuropathy were independent risk factors for severe hypoglycemia and protective higher body mass index measures. Severe hypoglycemic episodes in the past 3 months were associated with an increased risk of major cardiovascular events and cardiovascular and all-cause mortality, irrespective of the assignment of glycemic treatment group. Standard therapy also increased the risk of all-cause mortality following severe hypoglycemia.
