Effectiveness of home or hospital initiation of treatment at diagnosis for children with type 1 diabetes (DECIDE trial): A multicentre individually randomized controlled trial

Gregory JW, Townson J, Channon S, et al. - In this multicentre randomized controlled trial (January 2008/October 2013) of eight pediatric diabetes centers in England, Wales and Northern Ireland of 203 children, newly diagnosed with T1DM, were randomized to commence management at home (n = 101) or in hospital (n = 102) in order to ascertain whether management at home for beginning of insulin treatment and guidance of the child and family, would lead to enhanced clinical and psychological outcomes at 2 years postdiagnosis in children with newly diagnosed T1DM who were not acutely unwell. No variations in secondary outcomes at 24 months, apart from superior child self-esteem in the home-arm were noted. No home-arm children were admitted to the hospital during the beginning and no adverse events at that time were noticed. Moreover, no variations in insulin regimens between the two arms were there. Thus, in children newly diagnosed with T1DM across appropriate outcomes, no evidence of a disparity between the home-based and hospital-based beginning of care was discovered.
