Meinshausen M, et al. - This systematic review was performed to identify effectiveness and patient safety of platelet aggregation inhibitors in the prevention of cardiovascular disease and ischemic stroke in older adults. In addition, the authors aimed to develop recommendations on discontinuing platelet aggregation inhibitors (PAI) in this population if risks outweigh benefited. Among older people, the use of acetylsalicylic acid (ASA) for the primary prevention of cardiovascular disease (CVD) and the combination therapy of ASA and clopidogrel for the secondary prevention of vascular events could not be justified. In older people with atrial fibrillation, the use of oral anticoagulants instead of ASA could be recommended. Further high-quality studies with older adults were required.
