Effect of prophylactic embolization on patients with blunt trauma at high risk of splenectomy: A randomized clinical Trial

Arvieux C, Frandon J, Tidadini F, et al. - In this randomized clinical trial, researchers examined the rate of spleen rescue among patients with blunt trauma at high risk of spleen rupture receiving prophylactic splenic arterial embolization (pSAE) vs surveillance and embolization only if necessary (SURV). They randomized 133 patients with splenic trauma at high risk of rupture to undergo pSAE or SURV. A spleen rescue rate greater than 93% was achieved with both strategies in patients with severe splenic trauma. The groups exhibited no significant difference in the number of patients with an at least 50% viable spleen detected on a computed tomography scan at 1 month. In the surveillance group, embolization was done in many patients within a few days and significantly longer hospitalization was observed in many.
