Effect of no prehydration vs sodium bicarbonate prehydration prior to contrast-enhanced CT in the prevention of postcontrast acute kidney injury in adults with chronic kidney disease: The Kompas randomized clinical trial

Timal RJ, Kooiman J, Sijpkens YWJ, et al. - A randomized clinical trial was designed to evaluate the renal safety of omitting prophylactic prehydration before iodine-based contrast media administration in patients with stage 3 CKD. This trial performed at 6 hospitals in the Netherlands in which 523 individuals with stage 3 CKD were assigned randomly in a 1:1 ratio to receive no prehydration or prehydration with 250 mL of 1.4% sodium bicarbonate administered in a 1-hour infusion before undergoing elective contrast-enhanced CT between April 2013 and September 2016. Between January 2018 and June 2019, Data were examined. A total of 262 individuals were assigned to the no pre hydration group and 261 were assigned to receive prehydration. The examination of the primary endpoint was available in 505 patients (96.6%). The data showed that withholding prehydration did not compromise patient safety among individuals with stage 3 CKD undergoing contrast-enhanced computed tomography. The data of this research confirm the option of not giving prehydration as a safe and cost-efficient measure.
