Effect of intrathecal lipophilic opioids on the incidence of shivering in women undergoing cesarean delivery after spinal anesthesia: A systematic review and Bayesian network meta- analysis of randomized controlled trials

Subramani Y, Nagappa M, Kumar K, et al. - Via performing a network meta-analysis, researchers sought to ascertain how intrathecal lipophilic opioids affect the incidence of shivering in patients undergoing cesarean delivery (CD). They identified 21 studies consisting of 1,433 patients for inclusion (Control group: 590 patients in twenty-one studies; Fentanyl group:199 patients in seven studies; Sufentanil group: 156 patients in five studies; Meperidine group: 488 patients in ten studies). Outcomes suggest that with providing intrathecal (IT) fentanyl, the incidence of shivering decreases significantly in women undergoing CD under spinal anesthesia with no increase in maternal adverse events; this confirms its routine use in this patient population as a good choice. No decrease in the incidence of shivering was noted using IT sufentanil. Decreased incidence and severity of shivering was noted with using IT meperidine, but in association with significant nausea and vomiting.
