Effect of initial emergency room imaging choice on time to hip reduction and repeat imaging

Walker MR, et al. - As hip dislocations are highly morbid injuries necessitating prompt reduction and post-reduction assessment for fracture and incarcerated fragments, researchers investigated the association of choice of pre-reduction imaging with treatment of acute hip dislocations. They conducted a retrospective case-control at Single Academic Level I Trauma Center reviewing 123 hip dislocations. Patients were grouped into two: Group I had patients who had only a radiograph prior to reduction, vs Group II had patients with a pre-reduction computed tomography (CT) scan. The mean time to reduction was 74 minutes in Group I and 129 minutes in Group II, with rate of repeat CT scan of 0 vs 48 (81%), respectively, indicating the significance of initial trauma pelvic radiography prior to CT in the setting of suspected hip pathology to decrease time to hip reduction and unnecessary radiation exposure.
