Effect of hospital volume on 90-day mortality after radical cystectomy for bladder cancer in Spain

Llorente C, et al. - Mandatory hospital discharge forms of all subjects submitted to radical cystectomy (RC) due to bladder cancer in Spain during 2011–2015 (n =12,154 in 196 hospitals) were analyzed by the researchers in order to examine a 90-day mortality rate of RC for bladder cancer in a nationwide population-based study. In-hospital 30-, 60- and 90-day mortality was measured. Overall 90-day mortality rate was observed to be 6.5%. In hospitals doing more than 38 cases per year, lowest mortality rates were attained. With a 20.6% decrease per 10 extra RCs/year, the 90-day adjusted mortality rate had a correlation with annual average RC volume. The clinical variables correlated with greater mortality were High Charlson comorbidity index, advanced age, and open surgical approach. Hence, an inverse relationship between 90-day mortality and hospital volume was concluded. Moreover, within 90 days, high-volume hospitals attained lower mortality rate.
