Early vedolizumab trough levels at induction in inflammatory bowel disease patients with treatment failure during maintenance

European Journal of Gastroenterology & Hepatology Mar 10, 2019

Liefferinckx C, et al. - The impact of vedolizumab (VDZ) pharmacokinetics during induction on long-term treatment response was examined by researchers. This investigation focused on a single cohort of 103 patients treated with VDZ with inflammatory bowel disease. Investigators found that inflammatory bowel disease patients with a long-term response had higher trough levels (TLs) than patients in the treatment failure group at week 6 during VDZ induction. Compared to patients with mild or severe endoscopic activity, patients with mucosal healing in maintenance had higher TLs at week 6. Investigators confirmed that at induction an association between drug exposure and efficacy was found early on. This research emphasizes that preceding anti-TNFα exposure and positive cytoplasmic antibody serology perinuclear antineutrophil are important factors influencing induction VDZ TLs.
