Early hospital discharge following non-operative management of blunt liver and splenic trauma: A pilot randomized controlled trial

Kumar V, Mishra B, Joshi M, et al. - From the reviewed literature, this is the first randomized controlled trial investigating the feasibility and safety of early discharge in adult patients with blunt liver and splenic injury (BLSI) following non-operative management (NOM). After initial assessment and management, researchers randomized 60 patients aged 18-60 years with BLSI planned for NOM into 2 groups: Group A (test group; discharge day 3), and Group B (control group; discharge day 5). Patients, in majority, were males and aged less than 30 years. The most common mode of injury was road traffic injury. Findings suggest the possibility of performing safe discharge after 48 hours of in-hospital observation in adult patients undergoing NOM for BLSI, provided other injuries precluding discharge do not exist. Intensive monitoring, bedrest with bathroom privileges, physical examinations every 6 hours were recommended for the first 24 hours. They emphasize undertaking routine imaging (USG) before discharge and at 1 week after discharge to rule out latent pseudoaneurysms.
