Early detection of acute kidney injury in the perioperative period of liver transplant with neutrophil gelatinase-associated lipocalin

Lima C, de Paiva Haddad LB, de Melo PDV, et al. - Given patients undergoing liver transplant (LT) often suffer acute kidney injury (AKI) as a complication carrying high morbidity and mortality, researchers analyzed the pattern of urine and plasma neutrophil gelatinase-associated lipocalin (NGAL) rise during the perioperative period of LT and determined whether it holds value as a prognostic marker for AKI progression, requirement for dialysis and death. This study included 100 patients undergoing a liver transplant, of these, AKI based on the Kidney Disease: Improving Global Outcomes serum creatinine criterion was detected in 59. AKI development within 7 days post-surgery was well-predicted by urine NGAL values 6 h following surgery when a cut-off value of 136 ng/ml was applied, with a positive predictive value (PPV) of 80% with an AUC of 0.76. Also, the good predictive ability for AKI in the first week was displayed by plasma NGAL at 18 h post-LT, with a PPV of 81% and AUC of 0.74. Experts concluded that earlier prediction of AKI diagnosis in patients undergoing a liver transplant can be achieved by taking the plasma and urine NGAL elevation pattern in the perioperative period into account.
