Does the method of primary treatment affect the pattern of first recurrence in high-grade serous ovarian cancer?

Himoto Y, Cybulska P, Shitano F, et al. - Researchers examined the impact of the primary treatment approach (primary debulking surgery (PDS) vs neoadjuvant chemotherapy and interval debulking surgery (NACT-IDS)) on the pattern of the first recurrence in patients with completely cytoreduced advanced high-grade serous ovarian carcinoma (HGSOC). In this retrospective study, 178 patients with newly diagnosed stage IIIC–IV HGSOC were included; of these, 124 underwent complete gross resection during PDS and 54 underwent IDS. Data of baseline and first recurrence contrast-enhanced CT scans of these cases were obtained. Analyses suggest a potential influence of primary treatment approach on the pattern of the first recurrence observed on CT. More often, recurrence occurred in the same locations as the original disease among patients who underwent NACT-IDS. Recurrence was less frequently observed at the sites cleared previously by the surgery among patients who underwent PDS. The associations did not alter in relation to the route of adjuvant chemotherapy administration.
