Does computerized CT-based 3D planning of the humeral head cut help to restore the anatomy of the proximal humerus after stemless total shoulder arthroplasty?

Journal of Shoulder and Elbow Surgery Sep 21, 2020

Grubhofer F, Martinez ARM, Haberli J, et al. - This study was attempted to assess whether computerized CT-based 3D planning of the humeral head cut helps to restore the anatomy of the proximal humerus after stemless total shoulder arthroplasty. Researchers radiographically assessed a total of 100 consecutive stemless total shoulder arthroplasty (TSA) (67 males, 51 right shoulders, mean age of 62 ±9.4 years) applying pre- and postoperative standardized AP radiographs. After stemless TSA, restoration of proximal humeral anatomy using CT-based 3D planning was not precise. For overstuffing, a poorly conducted humeral head cut was the main reason which was seen in 88% of the cases with the inaccurate restoration of proximal humeral anatomy (RPHA). After stemless TSA, preoperative small humeral head diameter, low humeral head height, and varus angulated HNA are risk factors for poor RPHA.
