Brinck T, et al. - Via performing this trauma registry study, researchers examined if admission time (office hours vs non-office hours) influence the outcome of severely injured patients at a tertiary trauma center without an in-house trauma surgeon consultant available at all times. Further, they analyzed subgroups of patients presenting with a New Injury Severity Score (NISS) ≥ 25 and patients experiencing major bleeding. Severely injured patients (NISS > 15) with blunt trauma treated between 2006 and 2017 at a single institute, were assessed for causes of deaths and were classified as resulting from brain injury; exsanguination; multi-organ failure, adult respiratory distress syndrome, or sepsis; or other. They analyzed 1,853 patients including 497 (27%) who were admitted during office hours and 1,356 (73%) who were admitted during non-office hours. Outcomes revealed no correlation of arrival time with mortality among patients with severe blunt trauma treated at this tertiary trauma center. Thus, by investing in precise treatment protocols and continuous education, a standard of care can be maintained in this type of unit during non-office hours. However, the results do not implement to penetrating trauma injury patients.
