Bruckel J, et al. - Researchers examined the influence of accounting for early do-not-resuscitate (DNR) on hospital quality measures for acute myocardial infarction (MI). They analyzed 109,521 patients with acute MI from 289 hospitals via the California State Inpatient Database; 8.5% of patients of these had early DNR. In risk-adjustment models, including early DNR status substantially improved model fit and led to significant reclassification of hospital performance rankings for mortality and moderate reclassification for readmissions.Hence, the investigators recommended considering DNR status at hospital admission when reporting risk-standardized hospital mortality.
