Do antibiotic intramedullary dowels assist in eradicating infection in two-stage resection for septic total knee arthroplasty?

Zielinski MR, et al. - One hundred and nine consecutive patients who underwent two-stage treatment for periprosthetic knee infection were studied by the researchers to study the quantification of the treatment success with and without the use of antibiotic-eluding intramedullary (IM) dowels in two-stage treatment using static and articulating antibiotic cement spacers. Treatment success remained highest in patients with articulating spacers with or without IM dowels compared with static spacers with IM dowels of chronically infected poor hosts with compromised extremities. No enhancement was concluded by the authors in infection eradication with the use of IM dowels above and beyond of that observed for articulating spacers alone, including the worst cases involving chronically infected poor hosts with compromised extremities.
