Diffusion-weighted MRI assessment of the peritoneal cancer index before cytoreductive surgery

van 't Sant I, et al. - Researchers performed a prospective study at a tertiary referral center to assess the utility of diffusion-weighted MRI (DW-MRI) in selecting patients for cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS–HIPEC) among those with limited peritoneal metastases from colorectal cancer. Two radiologists evaluated the peritoneal cancer index (PCI) on CT-PCI and DW-MRI (MRI-PCI) in 49 evaluable patients. Readers 1 and 2 correctly staged 47 of 49 and 44 of 49 patients, respectively, and recognized all patients with the resectable disease with a PCI below 21 at surgery (sensitivity 100%). Findings thus suggested DW-MRI as a valuable non-invasive tool that may allow treatment selection in these patients.
