Diagnostic assessment of lower urinary tract symptoms in men considering prostate surgery: A noninferiority randomised controlled trial of urodynamics in 26 hospitals

Drake MJ, Lewis AL, Young GJ, et al. - This study was attempted to assess if a care pathway including urodynamics (UDS) would decrease surgery without increasing urinary symptoms. A pragmatic, noninferiority, randomized controlled trial was conducted to include men with bothersome lower urinary tract symptoms (LUTS), in whom surgery was an option, in 26 hospitals in England. Researchers randomly assigned a total of 820 men (median age 68 yr) (393 and 427 in the routine care (RC) and UDS arms, respectively). Individuals were allocated randomly in 1:1 ratio to RC diagnostic tests, or RC plus UDS. In this population, for the International prostate symptom score the UDS randomized group was non-inferior to RC but did not decrease surgical rates. The data exhibit that the routine use of UDS in the assessment of uncomplicated LUTS has a limited role and should be applied selectively.
