Diagnostic accuracy of portable chest radiograph in mechanically ventilated patients when compared with autopsy findings

Panizo-Alcañiz J, Frutos-Vivar F, Thille AW, et al. - Considering autopsy findings as the gold standard, researchers here evaluated diagnostic accuracy of portable chest radiograph in mechanically ventilated patients. In addition, they assessed the interobserver agreement among intensivists and radiologists. In this retrospective study, they assessed 422 patients who died in the ICU, underwent an autopsy, and had at least one portable chest radiograph 72 h prior to death over 22 years. Each chest radiograph was independently read by two intensivists and two radiologists. Findings suggest that in ventilated patients, portable chest radiograph has an overall sensitivity/specificity for pneumonia of 24% and 91%. For ARDS, portable chest radiograph has an overall sensitivity/specificity of 68% and 74% in ventilated patients. Radiologists vs intensivists exhibited higher interobserver agreement reading portable chest radiographs.
