Diagnosing adenomyosis with MRI: A prospective study revisiting the junctional zone thickness cutoff of 12 mm as a diagnostic marker

Tellum T, et al. - Via a single-center, prospective observational study that consecutively recruited 93 premenopausal women who suffered from a benign gynecological condition, from September 2014 to August 2016, the researchers determined the diagnostic exactness of a junctional zone (JZ) thickness of ≥ 12 mm and morphological features of the JZ in magnetic resonance imaging for the diagnosis of adenomyosis in a premenopausal study population. Fifty-seven (61%) women got adenomyosis confirmed by histopathology. No positive association of junctional zone thickness (JZmax) with adenomyosis diagnosis was found with no difference among those with and without adenomyosis and a cutoff of JZmax ≥ 12 mm. Among the assessed signs, the best correlation of the presence of an irregular JZ with adenomyosis was ascertained. Hence, in the present premenopausal study population, JZmax had no association with adenomyosis, however, direct signs of adenomyosis such as irregularities of the JZ gave a great diagnostic exactness.
