Diabetes mellitus–related all-cause and cardiovascular mortality in a national cohort of adults

Raghavan S, et al. - Using covariate-adjusted incidence rates and multivariable Cox proportional hazards regression, researchers retrospectively assessed all-cause and cardiovascular disease (CVD) mortality in relation to diabetes mellitus status and hemoglobin A1c (HbA1c) in 963,648 adults receiving care in the US Veterans Affairs Healthcare System from 2002 to 2014 and followed up for a mean duration of 8 years. Diabetes mellitus was present in 34% of participants. They noted 7.0 and 3.5 deaths/1000-person-years higher all-cause and CVD mortality among diabetics vs non-diabetics. Findings revealed a significant link between diabetes mellitus and all-cause and CVD mortality, however, compared to previous reports, a lower diabetes mellitus–related excess mortality has been reported in the contemporary era. With increasing HbA1c >6% to 6.9%, a gradient of mortality risk was observed. This implies that HbA1c continues to be an informative predictor of outcomes.
