Determining and achieving target limb-length and offset in total hip arthroplasty using intraoperative digital radiography

Debbi EM, et al. - Via involving 102 consecutive patients with unilateral hip osteoarthritis undergoing primary total hip arthroplasty (THA) in the lateral decubitus position, researchers introduced a method for determining and achieving target limb-length and offset utilizing digital radiographic (DR) measurements. Limb-length and offset were assessed preoperatively, intraoperatively and postoperatively on anterior-posterior pelvic radiographs. In 100% of patients, limb-length was within 5 mm of target measurements and in 97.1% of patients, offset was within 5 mm of targets. For most patients undergoing THA, target limb-length and offset goals could be determined. Targets on the contralateral side are not always comparable. Intraoperative DR will allow surgeons in a homogenous cohort of patients with unilateral hip osteoarthritis with excellent clinical outcomes to accurately achieve target limb length and offset to within 5 mm.
