Determinants of level Ib involvement in oral squamous cell carcinoma and implications for submandibular gland-sparing neck dissection

Subramaniam N, et al. - Experts performed a detailed audit of level Ib involvement in oral squamous cell carcinoma (OSCC) to evaluate the feasibility of submandibular gland-sparing in neck dissection, the rate of direct involvement by the primary tumors, the involvement of periglandular level Ib nodes, and their determinants were assessed. Gland-sparing neck dissection was safe in early tumors (pT1pN0–1), with a good chance of minimizing xerostomia without a need for radiotherapy. Although, the oncological safety is unclear, findings suggested larger tumors with no clear evidence of submandibular gland invasion or suspicious level Ib lymphadenopathy could be considered for gland preservation.
