Derivation and validation of a practical Bedside Score for the diagnosis of cholecystitis

Yeh DD, et al. - A practical Bedside Score was developed for the diagnosis of cholecystitis and its accuracy was tested against the Tokyo Guidelines (TG13). Symptoms, physical signs, ultrasound signs, and labs were scoring system candidates. Researchers used a random split-sample approach to develop and validate the new clinical score. They performed multivariable regression analysis using development data to identify predictors of cholecystitis. To ensure positive/negative predictive values (PPV, NPV), cutoff values were chosen of at least 0.95. The clinical prediction score for cholecystitis was identified to have an accuracy equivalent to TG13. Use of this score may streamline work-up by reducing the requirement for comprehensive ultrasound evaluation and CRP measurement and might shorten Emergency Department length of stay.
