Deep neuromuscular blockade for endolaryngeal procedures: A multicenter randomized study

Laosuwan P, Songarj P, Lapisatepun W, et al. - In this multicenter, randomized, parallel intervention trial involving 102 patients who had microscopic endolaryngeal surgery at four university hospitals, researchers contrasted the surgical condition between deep neuromuscular blockade (NMB) and moderate NMB. Participants in the study were randomized into moderate NMB (train-of-four 1-2) (M group) or deep NMB (post-tetanic count 1-2) (D group) with moderate or high doses of rocuronium, respectively. During the surgery, surgical rating conditions (SRCs) were assessed. In 49 patients in the D group and 43 patients in the M group, clinically acceptable SRC was observed. In the M group, the frequency of notable vocal fold movement was significantly higher than the D group. According to findings, deep NMB with high rocuronium doses combined with 4 mg/kg sugammadex for reversal during endolaryngeal surgery provided better SRC and anesthetic conditions than moderate rocuronium NMB with 2 mg/kg sugammadex.
