De-escalating adjuvant therapies in older patients with lower risk estrogen receptor-positive breast cancer treated with breast-conserving surgery: A systematic review and meta-analysis

Savard MF, Clemons M, Hutton B, et al. - Among older patients aged ≥50 treated with breast-conserving surgery for lower-risk breast cancer, the results of omitting radiation therapy (RT) or endocrine therapy (ET) were assessed via this systematic review. Randomized controlled studies and prospective cohort studies assessing omission of RT and/or ET vs RT plus ET in patients, were identified from Medline, Embase, and the Cochrane Register of Controlled Trials. From 3860 citations, 10 prospective studies fulfilled eligibility criteria. A reduction in 5- and 10-year in-breast tumor recurrence was conferred by adjuvant RT, vs no RT, however no impact was seen on survival. Per existing evidence, RT can be skipped in older patients with lower-risk disease. However, there is a need for more trials on the omission of ET in order to better inform treatment decisions.
