Daily adjustment of glucocorticoids by patients with adrenal insufficiency

Schöfl C, et al. - Since patients with adrenal insufficiency (AI) need to adjust glucocorticoid (GC) dosage in response to stressful events and disease to prevent life-threatening adrenal crisis (AC), researchers assessed patients with AI with regard to self-management. For this investigation, 80 patient diaries were collected and analyzed. There was a total of 34,074 patient days recorded. Of these, they documented 4,622 days with discomfort, and patients increased their GC dose on only 35% of those days. According to findings, severe discomfort, particularly in gastrointestinal infection, did not always lead to increased dose. Low symptom scores in some patients resulted in an unsuitable increase in GC. A pressing need for better methods of training is highlighted by these results. Continuing patient education and individualization could be helped by keeping daily records.
