D-dimer blood concentrations to exclude left atrial thrombus in patients with atrial fibrillation

Almorad A, Ohanyan A, Bentea GP, et al. - To exclude left atrial (LA) thrombus before cardioversion for non-valvular atrial fibrillation (AF), two D-dimer cut-offs [one fixed to 500 ng/mL (DD500) and the other adjusted to 10 times the patient’s age (DDAge)] were compared in the D-dimer blood concentrations to exclude LA thrombus in patients with AF study. Participants were 142 patients with non-valvular AF who were referred for pre-cardioversion transoesophageal echocardiography (TOE) to exclude LA thrombus. Experts noted a higher specificity of DDAge in indicating absence of LA thrombus vs that of DD500 (66.4% vs 50.4%). In this study, D-dimer cut-offs were shown to be efficacious to exclude LA thrombus in patients experiencing AF. As a consequence of age adjustment, there was a great increment in the proportion of patients in whom LA thrombus can be safely excluded and therefore prevented pre-cardioversion TOE.
