CT vs endoscopic ultrasound for differentiating small (2–5 cm) gastrointestinal stromal tumors from leiomyomas

Huh CW, et al. - Seventy-one patients with pathologically proven gastric subepithelial tumors were recruited by the experts in order to compare the diagnostic performance of CT vs endoscopic ultrasound for gastric subepithelial tumors smaller than 5 cm and to recognize important CT findings related to the diagnosis of gastrointestinal stromal tumors. The endoscopic ultrasound had a sensitivity and a specificity of 77.6% and 61.5%, respectively, PPV of 90.0%, NPV of 38.1%, and an accuracy of 74.6%. CT had a sensitivity and a specificity of 84.5% and 53.8%, respectively, with PPV of 89.1%, NPV of 43.8%, and an accuracy of 78.9%. For the diagnosis of gastrointestinal stromal tumors, multivariate analysis exhibited that the presence of intralesional necrosis and a long diameter (LD)-short diameter (SD) ratio of less than 1.5 were independent CT findings. Thus, for the diagnosis of gastric subepithelial tumors smaller than 5 cm, CT was concluded as efficient as endoscopic ultrasound. Tumors with intralesional necrosis and an LD-SD ratio of less than 1.5 may need further pathologic diagnosis.
