Coverage of immediate postpartum long acting reversible contraception has improved birth intervals for at risk populations

Liberty A, et al. - In view of the revised Medicaid policy in South Carolina to cover reimbursement for immediate postpartum long-acting, reversible contraception in 2012, researchers investigated how the Medicaid policy change affected the initiation of long-acting and reversible contraception (immediate postpartum and postpartum) within key populations. Further, they examined the variation in immediate postpartum long-acting and reversible contraception use by adequate prenatal care (> 7 visits), metropolitan location, and medical comorbidities. They also examined how immediate postpartum and postpartum long-acting, reversible contraception influenced inter-pregnancy interval of less than 18 months. A historical cohort study of live births was conducted among Medicaid recipients in South Carolina between 2010-2017, two years before and five years after the policy change. They identified 187,438 births to 145,973 women for inclusion. Observations revealed an increase in the odds of receipt of immediate postpartum long-acting and reversible contraception use postpolicy change. Immediate postpartum long-acting and reversible contraception was more frequent among women at risk of subsequent pregnancy and complications (inadequate prenatal care and medical comorbidities), following the policy change. Based on the findings, they suggest the necessity for improving access in rural areas.
