Cost-effectiveness of coronary artery bypass grafting plus mitral valve repair vs coronary artery bypass grafting alone for moderate ischemic mitral regurgitation

Ferket BS, et al. - In view of the findings from the Cardiothoracic Surgical Trials Network demonstrating no differences in left ventricular reverse remodeling at 2 years between patients with moderate ischemic mitral regurgitation randomized to coronary artery bypass grafting plus mitral valve repair (n = 150) or coronary artery bypass grafting alone (n = 151), researchers compared costs and quality-adjusted survival to address health resource use implications of these findings. For coronary artery bypass grafting plus mitral valve repair vs coronary artery bypass grafting alone, in-hospital costs were $59,745 vs $51,326, respectively. Its costs remained higher ($107,733 vs $88,583) and quality-adjusted life-years showed no difference, with 5.08 vs 5.08 at 10 years. Findings thereby suggest that for patients with moderate ischemic mitral regurgitation, the addition of mitral valve repair to coronary artery bypass grafting is unlikely to be cost-effective.
