Cost analysis of cytoreductive surgery and hyperthermic intraperitoneal chemotherapy and the risk factors for their increased cost in a public insurance healthcare system – Single centre study

Klos D, Kriváčková D, Melichar B, et al. - Researchers sought to determine the costs of CRS and HIPEC and treatment of the related postoperative complications and to recognize risk factors associated with an increase in the cost of CRS and HIPEC in the public healthcare system. Eighty patients who underwent CRS and HIPEC between February 2016 and November 2018 in the Department of Surgery, University Hospital of Olomouc, Czech Republic, were retrospectively evaluated. Observations revealed the postoperative morbidity rate of 50% and the mortality rate of 2.5%. For all 80 patients, the mean length of hospitalization and ICU admission was 15.44 ± 8.43 and 6.15 ± 4.12, respectively, and for 40 patients without complications, 10.73 ± 2.93 and 3.73 ± 1.32, respectively, and for 40 patients with complications, 20.15 ± 13.93 and 8.58 ± 6.92, respectively. Although the total treatment cost touched €606,358, the total reimbursement was only €262,931. This suggests no full compensation for the costs of CRS and HIPEC by the Czech public health insurance system. As per the multivariate analysis, increased costs were evident in independent correlation with blood loss of ≥ 1.000 ml and grade I–V Clavien-Dindo complications. The main limiting factor remain hospital losses for further improving these procedures.
