Contributing factors to early recurrence of ventricular fibrillation during out-of-hospital cardiac arrest: An observational retrospective study

Jost D, Ha VHT, Trichereau J, et al. - Given the recommendation of external electric shock (EES) for treating ventricular fibrillation (VF) in out-of-hospital cardiac arrest (OHCA) and the common occurrence of refibrillation within 1 minute postshock, researchers here examined refibrillation times and sought clinical and electrical factors linked with them. Based on the Paris Fire Brigade database, this retrospective observational study involved non-traumatic OHCA over 18 years of age who received at least one shock with an AED from Basic Life Support (BLS) rescuers; from these, a sample was selected randomly to determine the refibrillation-times. They identified 13,181 patients experiencing OHCA from January 2010 to January 2014; of these, AED data were assessed from 215 patients (590 shocks), 82.1% males, median age 61 [IQR: 52−75] years. Findings suggest that in non-traumatic OHCA, refibrillations occur mostly within 45-s postshock. The two contributing factors to time to refibrillation were age and post-shock hands-off time. In multivariate analysis, a shorter postshock hands-off time favored earlier refibrillation, as well as older age.
