Continuous hemodialysis with citrate anticoagulation and standard dialysate for managing acute kidney injury in patients with moderate to severe hyponatremia: A retrospective study

Hellman T, et al. - Whether continuous veno‐venous hemodialysis (CVVHD) with standard dialysate and citrate‐calcium anticoagulation is feasible in hyponatremic critically ill acute kidney injury (AKI) patients, was investigated in this retrospective study. Participants were 37 of the 493 critically ill AKI patients needing CVVHD who were admitted to intensive care unit and had hyponatremia (< 130mmol/l). CVVHD with citrate‐calcium anticoagulation and standard commercial dialysate was received by all patients. Findings revealed that plasma sodium level was effectively raised by CVVHD using standard citrate‐calcium anticoagulation. However, half of the patients exhibited excessively high plasma sodium correction. Standard citrate anticoagulation solutions offered a substantial sodium load.
