Complications and outcomes in diffuse large B-cell lymphoma with gastric lesions treated with R-CHOP

Kadota T, et al. - In this retrospective analysis of consecutive patients with newly diagnosed DLBCL with gastric lesions (diffuse large B-cell lymphoma [DLBCL]-GL) treated with R-CHOP between 2003 and 2014, researchers assessed the outcome after R-CHOP therapy including gastric complications as well as investigated risk factors for the complications. Gastric symptoms were present in 63 of 96 patients with DLBCL-GL. Six to eight cycles of R-CHOP was completed by 88 patients (92%). Findings revealed complete remission (CR) rate of 86%, and 3-year and 5-year overall survival rates of 80% and 73%, respectively. Complication rate was 8% (8/96). Bleeding and stenosis were seen in 7 and 3 patients, respectively. No case with gastric perforation was reported. During the first cycle of R-CHOP, 5 patients (5/7, 71%) bled. A lower R-CHOP completion rate (50%) and a lower CR rate (25%) were reported in patients with gastric complications vs those without complications. The only risk factor identified for gastric complications was a low serum albumin level at diagnosis. Stage IV was detected among six of the eight patients with complications.
