Complex endovascular aneurysm repair is associated with higher perioperative mortality but not late mortality compared with infrarenal endovascular aneurysm repair among octogenarians

Tran K, et al. - In this single-center retrospective review, clinical outcomes after complex endovascular aneurysm repair (EVAR) compared with infrarenal EVAR were evaluated specifically in a cohort of octogenarians. Using a database of consecutive patients treated with elective EVAR for abdominal aortic aneurysms (AAAs) between 2009 and 2015, researchers identified 103 patients (80 years of age or older). Among octogenarians, complex EVAR vs infrarenal EVAR was associated with greater perioperative mortality. However, the two procedure did not demonstrate significant differences regarding late outcomes, including the need for reintervention and all-cause mortality. Greater risk of late death after EVAR was identified in correlation with larger aneurysms and chronic kidney disease, regardless of AAA complexity. Hence while offering endovascular treatment to older patients, these patient-related factors should be considered.
