Comparison of whole-body DWI and 18F-FDG PET/CT for detecting intramedullary and extramedullary lesions in multiple myeloma

Chen J, Li C, Tian Y, et al. - Forty-nine patients with multiple myeloma who underwent whole-body (WB) DWI and PET/CT were recruited by the experts to contrast the ability of WB DWI and 18F-FDG PET/CT in discovering intramedullary and extramedullary lesions in multiple myeloma. In all regions of the body except the skull, scores were higher for WB DWI vs PET/CT, both in patients with newly diagnosed multiple myeloma and in formerly treated patients. Mean (± SD) percentages of plasma cells were significantly greater in group A vs group B. The mean number of extramedullary lesions detected by WB DWI was slightly greater in comparison with the mean number identified by PET/CT, however, there was no statistical variation. Thus, WB DWI is more sensitive for identifying intramedullary lesions compared with PET/CT in all regions except the skull, both in newly diagnosed patients and formerly treated patients and particularly in patients with a low percentage of plasma cells. For discovering extramedullary lesions, WB DWI has sensitivity comparable to that of PET/CT. Moreover, the utility of both modalities may give complementary knowledge.
